How do I request an official transcript?
If you completed a course at CGSC, you may request a transcript by downloading and completing the Document Request Form. This is a fillable PDF form and should be compatible with all PDF reader software. Once the form has been complete, please submit it via the “Submit Form” button provided (top right corner).
To avoid delays or denial of your request, please read all policy statements carefully before submission.
How should I address my request?
Be sure to include a complete mailing address that contains a numbered street address, building number, or P.O. Box. Our mailroom uses an automated system that requires a number in the second-to-last line of the address. If this is missing, your request will be returned.
It is also helpful to include the name of any counselor or university contact on the first line of the address.
Will the university accept a transcript from me?
Unless your university specifically requests that transcripts be mailed to the student, transcripts will only be sent directly from our office to the university, an employer (for job applications), or your S-1 for updating official records.
If your institution requires the transcript to be mailed to you, it will arrive in an envelope embossed and signed across the seal. To remain official, the envelope must be hand-carried to the receiving institution, which must open it.
Can I request a copy of my transcript for my personal records?
Yes! Download and complete the Document Request Form. If you would like a copy for personal use, please indicate this on the request form. Official and unofficial transcripts can be sent directly to you. Keep in mind that if the seal on the official transcript is broken, it becomes an unofficial transcript.
Important Notes About Delivery:
-
Official transcripts sent via email will have your SSN/ID# redacted for security reasons.
-
If you need your transcript sent to IPERMS, please include your S-1 or HR representative’s email or mailing address.
